The Project for an Ontario Women's Health Evidence-Based Report (POWER) Study has examined a broad set of evidence-based indicators bridging population health and health system performance. The Women's Health Equity Report provides actionable data for policy makers, providers and consumers in their efforts to improve health and reduce health inequities in Ontario.

In Chapter 12, "Social Determinants of Health and Populations at Risk" they synthesize prior analyses that examined the health of low-income, minority, and immigrant women, and report additional indicators of the social determinants of health and immigrant women's health. They discuss the health needs of populations at risk, how these differ among women and men in different groups, and highlight the role of the social determinants of health in the Ontario population.

They conclude the POWER Study with Chapter 13, "Achieving Health Equity in Ontario: Opportunities for Intervention and Improvement." In this chapter, they summarize and synthesize findings across all POWER Study reports. Based on the inequities documented throughout the POWER Study, they have developed the POWER Health Equity Road Map to help them move forward to the goal of achieving health equity in Ontario. The road map emerged from their analyses and broad community consultation and dialogue.
