The Public Health Agency of Canada is conducting a short evaluation regarding The Chief Public Health Officer's Report on the State of Public Health in Canada, 2012: Influencing Health - The Importance of Sex and Gender.

The evaluation consists of 12 questions, and should take about 10 minutes to complete.
